Inflammatory cells in urine or prostatic secretions do not correlate with the severity of the clinical symptoms. A nonspecific mixed inflammatory infiltrate composed of lymphocytes, plasma cells, and histiocytes is a common picture in chronic bacterial prostatitis. Viral and granulomatous prostatitis is present in persons with HIV infection, and the condition is culture-negative. Cytomegalovirus (CMV) is the most found virus causing prostatitis in HIV infected people. A randomized, double-blind, placebo-controlled trial was done to assess the efficacy and safety of mepolizumab versus placebo for severe bilateral nasal polyposis. Patients aged 18 to 70 years with recurrent nasal polyposis requiring surgery were chosen. Patients received 750 mg of intravenous mepolizumab or placebo every 4 weeks for a total of 6 doses in addition to daily topical corticosteroid treatment. A significantly greater proportion of patients in the mepolizumab group compared with the placebo group no longer required surgery at Week 25. There was a significant improvement in nasal polyposis severity VAS score, endoscopic nasal polyp score, all individual VAS symptom scores, and Sino-Nasal Outcome Test patient-reported outcome score in the mepolizumab compared with placebo groups. A retrospective study to review pediatric robotic rectopexy experience from 2012 to 2015 was done at a single institution. Four pediatric patients underwent a robotic rectopexy for rectal prolapse from 2012 to 2015. Three patients were male and one was female. The mean age was 15.5 years. Three patients were completed robotically, and one patient required conversion to an open procedure. The average postoperative length of stay was 3.25 days. There were no episodes of recurrent prolapse. The study concluded that Robotic rectopexy provided a safe, reliable, and short-term resolution of rectal prolapse in pediatric patients.
A study aimed to compare ventral mesh rectopexy (VMR) and pelvic organ prolapse suspension surgery (POPS) in the management of patients presenting with rectal prolapse was done. The study was a prospective cohort trial in which 120 female patients with complete rectal prolapse were included, 60 patients had had VMR and the other 60 had had POPS as surgical management for complete rectal prolapse. The patients were assessed 6 months postoperatively. There was no significant statistical difference regarding hospital stay and postoperative pain. Operative time was significantly shorter in POPS in comparison with VMR. VMR showed slight improvement regarding constipation and continence scores; however, this was statistically significant. VMR showed fewer complications compared to POPS. The study concluded that POPS is comparable to VMR in the management of rectal prolapse and in improving the ODS symptoms. The condition affects nerve fibres and skin, causing burning pain that lasts long after the rash and blisters of shingles disappear. The chickenpox (herpes zoster) virus causes shingles. The risk of developing postherpetic neuralgia gets more with age, it is manifested mostly in people older than 60. There’s no cure, but treatments can ease symptoms. In most cases, postherpetic neuralgia gets better over time. Signs & symptoms It is generally limited to the area of skin where the shingles outbreak first occurred — most commonly in a band around the trunk, usually on one side of the body. Signs and symptoms might include: Pain that lasts for more than three months after the shingles rash has been cured. The associated pain is described as burning, sharp and jabbing, or deep & aching. Sensitivity even to light and soft touch. People are not able to suffer even the touch of clothing on the affected skin (allodynia). In rare cases, an itchy feeling or numbness. Pathophysiology Varicella-Zoster Virus is a highly contagious DNA virus that remains latent within the sensory ganglia following the resolution of chickenpox, which usually occurs during childhood. During Herpes Zoster, Varicella Zoster Virus is reactivated, travels back along the affected neurons away from the sensory ganglia, and propagates in the epidermis. A hallmark of Herpes Zoster is that it is typically unilateral (i.e., not crossing the midline), and in most cases, only a single dermatome is affected. The erythematous maculopapular Herpes Zoster rash is usually accompanied by pain and dysesthesia. This rash develops into clear vesicles similar to the ones in chickenpox. Then, within around two days, pustules form, which then ulcerate, and finally scab over. Scabs fall off in 2–3 weeks and scarring may occur. Post Herpetic Neuralgia occurs in the same dermatomes as the Herpes Zoster rash and stems from damage to peripheral and central neurons that may be a by-product of the immune/inflammatory response that accompanied Varicella-Zoster Virus reactivation and migration. When damaged, peripheral and central nerve fibres may develop a lower threshold for action potentials, discharge spontaneously, and exhibit disproportionate responses to stimuli, resulting in peripheral sensitization and pain without painful stimuli (allodynia) Patients with PHN experience three major types of pain: 1) constant pain without a stimulus (often described as burning, aching, or throbbing), 2) intermittent pain without a stimulus (often described as stabbing, shooting, or electric shock-like), and 3) pain brought on by a stimulus but is disproportionate to the stimulus (hyperalgesia) enduring for at least 3 months after healing of the Herpes Zoster-related skin rash. In addition, patients may experience a variety of abnormal sensations (dysesthesias or paraesthesia’s). Diagnosis Physical examination Treatments No single treatment relieves postherpetic neuralgia for everyone. Mostly, a combination of treatments is needed to reduce the pain. Lidocaine skin patches – small patches like bandages that contain the topical pain-relieving medication lidocaine. The patches are applied, directly to painful skin to deliver temporary relief. Capsaicin skin patch – It uses a patch with a numbing medication on the affected area. Anticonvulsants – Certain anti-seizure medications, including gabapentin and pregabalin, can lessen the pain of postherpetic neuralgia. These medicines have remarkable side effects too. Antidepressants – Certain antidepressants, such as nortriptyline, amitriptyline, duloxetine and venlafaxine, affect key brain chemicals that play a role in both depression and how the body interprets pain. In everyday life, people encounter a number of smaller creatures may it be a germ or a parasite. Man’s life on earth is greatly influenced by the surrounding system including these organisms. Some of them live inside us in a friendly way but some of them cause potential harm to us, in the form of diseases. Pinworms are tiny, narrow worms white in colour and less than 2cm long. They are small parasites that live in the colon and rectum. Usually, people get affected when they swallow the worm eggs which hatch inside the human intestine. Female worms travel out of the infected person’s anus to lay eggs, mostly at night, causing severe itching on the area. Along with symptoms in digestive system, pin worms can cause chronic anaemia and other disorders. Strict personal hygiene measures are needed along with medications for a complete cure. Signs & symptoms Itching around the anus, especially at night Difficulty to fall asleep or disturbed sleep Irritability Teeth grinding Restlessness Occasional stomach pain, nausea & vomiting In girls, itching in the vaginal area and rarely vaginal discharge Causes Inadequate hygiene is the cause of pinworm infestation. Eggs are transferred from the anus of an infected person to either their own mouth, re-infecting themselves, or another surface. If other persons touch that contaminated surface and then touch their mouth, they can become infected by ingesting the eggs. Pathophysiology The primary symptoms of pinworm (Enterobius vermicularis) infection include pruritus or a prickling sensation in the perianal area, which is produced when a gravid female pinworm migrates to the anal area and inserts her pin shaped tail into the mucosa for depositing ova, usually at night time. E vermicularis is seen in the small intestines, mainly the ileocecal region. The movement of the female and the ova cause intense local itching. Ova may survive for up to 3 weeks before hatching. The hatched larvae can then migrate back into the anus and lower intestine, causing retro infection. Embryonated eggs may be released into the anus, air or onto fomites (e.g., bedding, clothing, toys, paper) or onto hands and then placed directly into the mouth and ingested (autoinfection), after which they settle in the small intestines. Pinworms that inhabit the caecum and adjacent areas typically cause no symptoms. Diarrhoea due to inflammation of the bowel wall can occur during acute infection. Risk factors for pinworms include living with a person who is egg-positive, not washing hands before eating, and poor personal or group hygiene. Diagnosis Identification of the worm or its eggs. Worms can sometimes be seen on the skin near the anus or on underwear or sheets about 2 to 3 hours after falling asleep. Pinworm eggs can be collected and examined using the “tape test” as soon as the person wakes up. Treatments Medications like mebendazole, pyrantel pamoate, or albendazole. Prognosis Pinworm infestation is not usually fatal; death and morbidity happen from secondary infection. Complications Complications are rare but they may include:
