A keloid sometimes referred to as a keloid scar, is a scar that rises quickly above the rest of the skin. They can be simply called scars that don’t know when to stop. Keloids are firm, irregular in shape, pink, purple or red, itchy bumps a smooth top that tend to enlarge progressively and appear at the site of previous skin damage. Unlike scars, keloids do not regress over time. It is observed that people with darker skin are typically more predisposed to develop keloids. Simply cutting out a keloid is likely to result in an even larger keloid developing at the excision site. People with a tendency to form keloids should avoid cosmetic surgery. Signs & symptoms Keloids are raised areas of skin rashes and look shiny and dome-shaped, ranging from pink to red. Some keloids become quite large and ugly. Aside from causing potential cosmetic problems, these scars tend to be itchy, tender, or even painful to the touch. Keloids develop most often on the chest, back, shoulders, and earlobes. Causes The exact cause of keloid is not known. Hereditary factors play a role. A mutation in a gene known as NEDD4 gene may indicate that a person has a predisposition to keloid formation. Pathophysiology Though the pathophysiology of keloid and hypertrophic scars is not completely known, various cytokines have been implicated, including interleukin (IL)-6, IL-8, and IL-10, as well as various growth factors including transforming growth factor-beta and platelet-derived growth factor. Keloid tissue is mostly composed of disorganised type 1 and 111 collagens, containing pale-staining hypocellular collagen bundles with no nodules or excess myofibroblasts. Diagnosis A direct physical examination can confirm the diagnosis of a keloid. A keloid has a characteristic microscopic appearance and may be distinguished from a hypertrophic scar and a dermatofibroma. Treatments The methods now available to treat keloids are as follows:
Prognosis Small keloids can be effectively treated using a variety of methods. Generally, a series of injections of steroids into the area is the simplest and safest approach. The patient needs to understand that the keloid will never entirely disappear but is likely to become less symptomatic and flattened. Larger lesions are more difficult to treat. Complications Severe complications like carcinoma, are seen very rare. In persons with diabetes, neurological disorders and joint diseases, keloids can cause severe issues like inflammation, numbness, or limited mobility. The most common complication people choose to take keloid treatment is a change in appearance; a cosmetic issue. Discomfort, pain or tenderness of the keloid. Irritation from rubbing or friction on clothes can cause flare-ups. Limited mobility: When the keloid is extensive after burns or injuries, keloids can limit the mobility of the body parts due to contractures of the skin and its tissues. In short, millions of patients are suffering from the debilitating effects of joint pain for which there is no satisfactory treatment. Signs & symptoms Pain in the joints Stiffness Swelling Painful movements/limited range of movements Causes Joint pains can develop due to numerous reasons. It can also present as an associated symptom of another main disease. The main causes of joint diseases in clinical practise are: Injury Wear and tear Deficiency disorders mainly Calcium deficiency Degeneration (Senile arthritis) Osteoporosis Rheumatoid arthritis Osteoarthritis Gout Pathophysiology As the term ‘joint pains’ is a term denoting a wide variety of diseases, single pathophysiology is not there for it. The pathophysiology is different and specific for each disease that causing joint pain. This review examines the sensory innervation of diarthrodial joints and discusses the neurophysiological processes that lead to the generation of painful sensations. During inflammation, joint nerves become sensitized to mechanical stimuli through the actions of neuropeptides, eicosanoids, proteinase-activated receptors, and ion channel ligands. The contribution of immunocytes to arthritis pain is also reviewed. Finally, the existence of an endogenous analgesic system in joints is considered and the reasons for its inability to control pain are postulated. The neurobiological processes initiated in the joint give rise to neural signals and then decoded by the central nervous system into pain perception. Joint innervation and nociception Most of the weight-bearing joints are richly innervated by sensory and sympathetic nerves. Postganglionic sympathetic fibers terminate near articular blood vessels, where they regulate joint blood flow through varying degrees of vasoconstrictor tone. The primary function of sensory nerves is to detect and transmit mechanical information from the joint to the central nervous system. Large diameter myelinated nerve fibers encode and transmit proprioceptive signals, which can be interpreted as being either dynamic (movement sensations) or static (position sense). Pain-sensing nerve fibers are typically less than 5 μm in diameter and are either unmyelinated (type IV) or myelinated with an unmyelinated ‘free’ nerve ending (type III). These slowly conducting fibers typically have a high threshold and only respond to noxious mechanical stimuli, and as such are referred to as nociceptors. In the rat and cat, 80% of all knee joint afferent nerve fibers are nociceptive, suggesting that joints are astutely designed to sense abnormal and potentially destructive movement. Nociceptors are located throughout the joint, having been identified in the capsule, ligaments, menisci, periosteum, and subchondral bone. The most distal segment of type III and type IV afferents is devoid of a myelin sheath and perineurium, and it is believed that this is the sensory region of the nociceptive nerve. Transmission electron microscopy revealed an hourglass shape repeating pattern along the length of type III and type IV nerve terminals, and the multiple bulbous areas exhibit the characteristic features of receptive sites. It is within these ‘bead-like structures on the terminals of ‘free’ nerve endings that joint pain originates. The question of how a painful mechanical stimulus is converted into an electrical signal that can then be propagated along sensory nerves to the central nervous system is still unclear. The exposed nature of sensory ‘free’ nerve endings means that the axolemma of these fibers is probably subjected to significant stretch during joint movement. The recent identification of mechano-gated ion channels on type III and type IV knee joint afferents by electrophysiological means provided the first insight into the physiological mechanisms responsible for mechano-transduction in joints. The present theory is that movement of the joint generates shear stresses on the axolemma of the ‘free’ nerve endings, resulting in the opening of mechano-gated ion channels. This leads to a depolarization of the nerve terminal and the generation of action potentials, which are subsequently transmitted to the central nervous system where they are decoded into mechano-sensation. If a noxious movement is applied to the joint, the firing rate of the afferent nerve increases dramatically and the central nervous system interprets this nociceptive activity as pain. In addition to this cognitive aspect of arthritis pain, there is also an affective or emotional component to the disease. Patients who suffer from chronic arthritis pain exhibit clinical signs of depression and anxiety that appear to have a physiological basis. Some studies found that noxious mechanical stimuli applied to acutely inflamed joints had an excitatory effect on the firing rate of neurons in the central nucleus of the amygdala. These studies provide the first electrophysiological evidence that the amygdala is involved in transforming nociceptive information arising from arthritic joints into an emotional, painful experience. Sometimes physical therapy sessions to strengthen the muscles around the joint will make it more stable and ensure the best mechanical movements. Working with a physical therapist can help avoid injuries or further worsening of the condition.
Injecting medications directly into the joint might help in certain situations where not many joints are involved. The two most common injections are corticosteroids and lubricants. Corticosteroid injections can help arthritis and other inflammations of the joints, mainly weight-bearing ones. They usually need to be repeated every few months. Lubricants that are similar to the synovial fluid.
Arthroscopic joint surgery: It allows the surgeon looks inside the joint through a few small holes and an optic fiber camera. The surgeon can repair many of the injuries and remove small pieces of loose bones or cartilage. This is a common outpatient procedure. Partial joint replacement: The surgeon replaces the damaged portions of the joint with artificial parts. Only part of the joint is replaced in this procedure. Total joint replacement: In this procedure, the knee is replaced with an artificial joint. Prognosis The prognosis of the joint pain depends upon many factors like age of the patient, disease-causing the joint pain, any associated illness like diabetes, etc. Usually, acute inflammatory joint pains have a good prognosis with proper treatment. Chronic arthritic pains may need long-term treatment and recurrence is common. The name jaundice came from the French word Jaune meaning yellow. Jaundice or icterus is a term used to describe yellowish discoloration of the skin, mucous membrane, body fluids, and sclera of the eye. It happens due to the increased amount of bilirubin in blood & body fluids and the colour may vary from yellow to brown. Though the term is familiar as Neonatal jaundice, which affects newborn babies, the disease can affect people of all ages. It is usually an indicator of an underlying issue like diseases of the liver, obstruction in the bile duct or an infection. Jaundice occurs when there is a derangement in the metabolism or excretion of bilirubin. Thus, jaundice itself is not only a disease but an indicator of an underlying issue. Signs & symptoms Yellowish tinge to the skin, mucous membranes, body fluids and sclera. Pale stools. Dark or yellow coloured urine. Itching of the skin, partial or all over the body. Fatigue & weakness Abdominal pain Weight loss Nausea& Vomiting Fever & chills. Loss of appetite. Diarrhoea & rectal bleeding. Headache. Confusion. Swelling in the extremities. Swelling in the abdomen.
Causes
It can be divided into three based on where the disruption/dysfunction occurs.
Haemolytic anaemia/Jaundice can happen due to diseases like: Sickle cell anaemia Glucose-6-phosphate dehydrogenase deficiency (G6PD) Hereditary spherocytosis Malaria Thalassemia Some drugs or toxins, and Autoimmune disorders.
Conditions with a hepatic cause of jaundice include: Acute or chronic hepatitis (commonly viral Hepatitis A, B, C, D, E] or alcohol-related) Liver cirrhosis (caused by various conditions including alcohol toxicity) Drugs or other toxins Crigler-Najjar syndrome Autoimmune disorders Gilbert’s syndrome Carcinoma of liver
Conditions that can cause post-hepatic jaundice include Cholecystitis Carcinoma of the pancreas, gall bladder or bile duct Stricture of the bile ducts Cholangitis Pancreatitis Certain worms/parasites like liver flukes Pathophysiology Jaundice in adults is caused by various medical conditions that affect the normal metabolism or excretion of bilirubin. Bilirubin is normally formed from the daily breakdown and destruction of red blood cells in the bloodstream, which release haemoglobin as they rupture. The haeme portion of this haemoglobin molecule is then converted into bilirubin, which is transported in the bloodstream to the liver for further metabolism and excretion. In the liver, the bilirubin is conjugated and made more water-soluble, it is excreted into the gallbladder for storage and then eventually passed into the intestines, where a portion of the bilirubin is excreted in the faeces, while some are metabolized by the intestinal bacteria and excreted in the urine. Jaundice with a pre-hepatic cause is due to the excessive destruction (haemolysis) of red blood cells. This causes a quick rise in bilirubin levels in the bloodstream which disturbs the liver’s capability to metabolize the bilirubin, and consequently the levels of unconjugated bilirubin increase. Jaundice caused during the hepatic phase can arise from abnormalities in the metabolism and/or excretion of bilirubin. This can lead to an increase in both unconjugated and/or conjugated bilirubin levels. In certain cases, imaging studies like
are taken to confirm the diagnosis.At times, further invasive tests like endoscopic retrograde cholangiopancreatography (ERCP) or a liver biopsy will be needed to determine the cause of jaundice. Treatments The treatment is decided according to the cause of jaundice and severity. Some patients will require hospitalization, whereas others can be managed as outpatients at home.
Prognosis The prognosis of jaundice solely depends upon the cause or underlying condition. Some jaundice patients recover so well that they indicate an excellent prognosis whereas some severe cases can be fatal despite medicines and surgical invasion. The development and severity of complications will also determine an individual’s prognosis, as well as underlying comorbidities (other diseases present). Therefore, a case-by-case assessment is necessary to determine a more accurate prognosis. Some persons recover fully without any complications whereas some people carry a lifelong set of complications after a jaundice attack. It depends upon the general health of the person and the cause leading to jaundice. Complications A few of the potential complications include:
