A peptic ulcer is a sore that develops on the lining of the stomach, small intestine or oesophagus. A peptic ulcer when developed in the stomach is called a gastric ulcer. A duodenal ulcer when it develops in the first part of the small intestine (duodenum). An oesophageal ulcer when it is manifested in the lower part of the oesophagus. Ulcers develop when stomach acid damages the lining of the digestive tract. Common causes are the bacteria H. Pylori and anti-inflammatory pain relievers including aspirin. Upper abdominal pain is the main sign. Treatment options include medication to decrease stomach acid production. If it is caused by bacteria, antibiotics may be beneficial. Signs & symptoms The most common symptom is a burning sensation or pain in the middle of the abdomen between your chest and belly button. Typically, the pain will be more intense when the stomach is empty, and it can last for a few minutes to several hours. Other common signs and symptoms of ulcers include:
Causes Stomach ulcers are mostly caused by any one of the following: H.pylori infection long-term use of nonsteroidal anti-inflammatory drugs (NSAIDs), such as aspirin, ibuprofen, or naproxen Zollinger-Ellison syndrome (a rare medical condition) Pathophysiology Peptic ulcer disease (PUD) shows a discontinuation in the inner lining of the gastrointestinal tract because of excessive gastric acid secretion or pepsin. It can reach upto the muscularis propria of the gastric epithelium. In short, peptic ulcers are defects in the mucosa that extend through the muscularis mucosa. Normally, a physiological balance is there between gastric acid secretion and gastroduodenal mucosal defence. Ulcer develops when this balance is disrupted. Aggressive factors, such as nonsteroidal anti-inflammatory drugs (NSAIDs), H pylori infection, alcohol, bile salts, acid, and pepsin, can alter the mucosal defence by allowing back diffusion of hydrogen ions and subsequent epithelial cell injury. The defensive mechanisms are tight intercellular junctions, mucus, bicarbonate, mucosal blood flow, cellular restitution, and epithelial renewal mechanism. Diagnosis Medical history Blood test, stool test, or a breath test to rule out H.pylori infection Barium swallow test Endoscopy (EGD) Endoscopic biopsy Treatments It’s important to promptly treat an ulcer. In most cases, it is known as involuntary and uncontrollable tremors. This disease doesn’t kill the patient instantly but life becomes worse day after day, moment after moment. It is a complex disease that has many different systems so that not everyone with the condition suffers from the same problems. Parkinson’s disease is named after the doctor who first wrote about the disease, in 1817. Parkinson called it, “The Shaking palsy,” or “paralysis agitans.” In those times, the term “agitans” referred to tremors. “Palsy” meant weakness and “paralysis” meant paralyzed. In the early 1960s doctors understood the importance of the brain chemical, dopamine (di-ortho-phenyl-alanine), followed shortly thereafter by the first substantive treatment for Parkinson’s disease, L-Dopa. The treatment for Parkinson’s Disease was very poor before this. L-Dopa wasn’t very popular at first, even though it had a scientific rationale, because it caused so much nausea and vomiting. Carbodopa was developed then, to prevent vomiting. The combined medication, carbidopa/levodopa, is marketed as Sinemet. This drug is the single best drug we have for treating the symptoms of Parkinson’s disease. PD is common. After Alzheimer’s disease, it is the second most common progressive, neurological disorder. The more that Parkinson’s Disease is studied, the more intricate our findings are. Although most people think of it as a disorder of movement, posture and balance, it may also affect the behaviour of a person. Quality of life has become a central focus as we deal with a disease we cannot yet cure. When we talk about treating Parkinson’s disease, we currently only mean treating the symptoms. The actual disease is an attack on the nerve cells in the brain and maybe, outside the brain. Current medications have been shown effective to improve symptoms, meaning that by helping to restore a more normal chemical balance in the brain, we improve the tremor, stiffness, slowness, mobility, etc, but we are not yet successful in the process that is causing the damage. Causes The exact cause is not yet known. Pathophysiology The most prominent pathological findings in Parkinson’s disease are degeneration of the darkly pigmented dopamine neurons, particularly evident in a part of the substantia nigra called the pars compacta. Significantly, the loss of dopamine in the pars compacta increases the overall excitatory drive in the basal ganglia, disrupting voluntary motor control and causing the characteristic symptoms of PD. There is also a loss of dopamine in the neostriatum and the presence of intracellular inclusion bodies known as Lewy bodies. Diagnosis Not a single and specific test is there to diagnose Parkinson’s disease. Following methods when combined with signs & symptoms help in diagnosis.
Treatments Treatment starts with levodopa (L-dopa). But it can cause stomach upset, so it is combined with another medicine called carbidopa to control these side effects. The combination drug is carbidopa-levodopa (Parcopa, Rytary, Sinemet). Prognosis Parkinson’s disease is not a fatal one, but the condition can cause great strain on the body and can make some people more vulnerable to serious and life-threatening infections. But with advanced treatment, most Parkinson’s patients now have a normal or near-normal life expectancy. Complications Cognitive Problems like thinking and reasoning problems. In normal conditions, Vaata is the controller of body & mind, it is responsible for all the movement. Like air, without any visible form, Vaatadosha travels all the body, fills up all the empty spaces and controls & regulates the functions of other doshas and dhatus (bodily tissues). When this Vaata dosha is vitiated, abnormal movements can occur as a symptom. When it happens in the form of tremors, it is called Kampavaata. The total concept of Parkinson Disease in Ayurveda is different from the Western view. Ayurveda considers PD as a metabolic disorder of chronic patterns. The impairment of Vata is caused due to deranged digestive system. The vitiation of Vata as described under the head Nidana, clearly communicate the concept of Ayurveda and Ayurvedic Treatment for Parkinson’s aim to correct the basic disorder in the body. Nidana Vaatakopa can happen mainly in two ways.

The person will have the feeling of the need to pass urine many times during the day and night, and at times, unintentional loss of urine (urgency incontinence). Rather than being a disease, it causes embarrassment, isolation, and limitation to work and social life. The good news is that a brief evaluation can determine whether there’s a specific cause for the overactive bladder symptoms. In most cases, symptoms of an overactive bladder can be managed with simple behavioural strategies, such as dietary changes, timed voiding and bladder-holding techniques using pelvic floor muscles. Causes The exact cause of an overactive bladder is mostly unknown. But several conditions may contribute to signs and symptoms of overactive bladder, including:

Pathophysiology Three main factors have been proposed regarding the cause of overactive bladder: myogenic (related with muscles), neurogenic (related with nerves) and urotheliogenic (related to the lining of urinary organs). Disturbance of any of the three factors or a combination of these factors can attribute to an overactive bladder. Normally, as the urinary bladder fills, nerve signals sent to the brain eventually trigger the need to urinate. When urinated, these nerve signals coordinate the relaxation of the pelvic floor muscles and the muscles of the urethra (urinary sphincter muscles). The muscles of the bladder tighten (contract), pushing the urine out. When this process and the contraction mechanism becomes completely deranged, involuntary bladder contractions occur leading to an overactive bladder. It happens when the muscles of the bladder start to contract involuntarily even if the volume of urine in the bladder is low. Such involuntary contractions create an urgency to urinate. Metabolic derangement, bladder outlet obstruction and inflammation can increase the excitability of the nerve, the detrusor muscle and alter the sensory and barrier functions of the urothelium. The detection of proteins in the urine such as NGF, PGE2, and proinflammatory chemokines leads to the pathophysiology of overactive bladder and offer novel diagnostic biomarkers of overactive bladder. It is characterized by itchy, non-infectious rashes on the arms and legs. It consists of small, flat on top, pink- or purple-coloured bumps with many sides. Considered to be an autoimmune disease, it typically occurs in people over the age of 30 years. It occurs more in females than in males. People with liver diseases, such as hepatitis C or cirrhosis are more susceptible to lichen planus. The mouth is a common site of inflammation and clinical manifestation of lichen planus and the condition is known as oral lichen planus. Signs & symptoms Itchy, discoloured patch or patterns of shiny, raised, red, pink, or purple, flat-topped bumps on the skin inside cheeks & tongue. Papules, approximately 3 to 5 mm in diameter. White streaks on the papules are called Wickham’s striae. Intense itching can develop, mostly at night. The first attack may continue for weeks or months, while recurring sessions can continue for years. Recurrence is very common in cases of oral lichen planus. Oral lichen planus
Causes The exact cause of lichen planus is not known yet. It is considered to be an autoimmune disorder, where the immune system mistakenly attacks healthy tissues. Hereditary factors do not play any role in this condition. It is not an infectious condition It is not related to malnourishment/deficiencies. But some spicy foods, citrus juices, etc. can aggravate open sores in the mouth. Some reasons for lichen planus are believed to be the following: Medications – Lichen planus can develop as a reaction to medicines:
Mercury tooth fillings. Grinspan’s syndrome – characterized by hypertension, diabetes, and oral lichen planus. Pathophysiology The classic histopathology of Oral Lichen Planus is liquefactive degeneration of the basal cell accompanied by apoptosis of the keratinocytes, a dense band-like lymphocytic infiltrate at the interface between the epithelium and the connective tissue, focal areas of hyper-keratinized epithelium and occasional areas of atrophic epithelium where the rete pegs may be shortened and pointed (a characteristic known as sawtooth rete pegs). Eosinophilic colloid bodies or Civatte bodies, which are degenerating keratinocytes, are often visible in the lower part of the surface epithelium. Degeneration of the basal keratinocytes and disruption of the anchoring elements of the epithelial basal membrane and basal keratinocytes weaken the epithelial connective tissue interface. As a result, histologic clefts (Max–Joseph spaces) can form and blisters on the oral mucosa develop. Diagnosis Physical examination due to its appearance. A punch biopsy of skin Treatments Lichen planus is not a curable condition. In most cases, it is a harmless disorder causing no or negligible problems. It is important to maintain good oral hygiene despite the pain. Therapy includes antihistamines, phototherapy, or steroid creams/tablets. Antihistamines are used to manage itching in severe cases. For lichen planus that occurs in the mouth, steroid lozenges are used or mouthwashes if mouth ulcer symptoms become uncomfortable. Corticosteroids will be needed in severe cases as tablets. Prognosis Usually, mild lichen planus on the skin clears up on its own in some months to years. Moderate cases subside within some weeks to months with treatment. Some cases where the disease affects mucous membranes are resistant to treatment and prone to recur. Complications Permanent brown or grey marks on the skin. Rarely, recurrent and persistent aberrations on the skin and mucosa increase the chance of getting oral cancer in patients with lichen planus. Mucus in the body is natural and a very important part of body functions. Tissues produce mucus to the line and protect the areas including the mouth, nose, sinuses, throat, lungs, and gut. Mostly, the mucus is clear and thin. However, some factors can increase mucus consistency. Mucus can even change in colour in some diseases. The content of the stool is also an indicator of health or disease. Mucus present in stools can be a sign of a serious underlying condition. Signs & symptoms Presence of mucus in the stool Associated signs & symptoms depend upon the underlying cause. Gastric symptoms that may be present with mucus in stool include: Abdominal cramps or pain Distension of abdomen Bloating or belching The foul or abnormal smell of stools Changes in colour & consistency of stools Diarrhoea Faecal incontinence or urgency Gas trouble Nausea Vomiting Painful defaecation Causes Dehydration and constipation can cause the loss of mucus. Diet-related causes – Food allergies such as those associated with nuts, lactose, or gluten, can cause mucus in the stool.Pathophysiology Diarrhoea is the reversal of the normal net absorptive status of water and electrolyte absorption to secretion. Such a derangement can be seen as either osmolar or secretory pathophysiology. Mucus present in the stool mostly shows the secretory type of diarrhoea. In the typical active secretory state, enhanced anion secretion (mostly by the crypt cell compartment) is best exemplified by enterotoxin-induced diarrhoea. In secretory diarrhoea, the epithelial cells processes are changed mostly into active secretion. The most common cause of acute-onset secretory diarrhoea is a bacterial infection of the gut. Several mechanisms may be at work. After colonization, enteric pathogens may adhere to or invade the epithelium; they may produce enterotoxins (exotoxins that elicit secretion by increasing an intracellular second messenger) or cytotoxins. They cause cytokines to attract inflammatory cells, then leading to the activated secretion by inducing the release of agents such as prostaglandins or platelet-activating factors. Features of secretory diarrhoea include a high rate of evacuation, a lack of response to fasting, and intact nutrient absorption.
