Pathophysiology Degeneration of the rotator cuff tendon results from a variety of intrinsic factors including but not limited to age-related degeneration, inflammation, vascular changes, and oxidative stress. Among these elements, the key factor leading to rotator cuff weakness and degeneration is aging. Rotator cuff injury starts from trauma. Macro-trauma causes an acute tear, which is seen generally a younger patient resulting in a complete tear. Micro-trauma causes tendon degeneration and with insufficient healing, leads to degenerative tears. Typically, acute tears happen in younger patients, and degenerative tears occur in older patients. However, a smaller amount of force is needed to cause a complete tear if there is sufficient tendon degeneration. Diagnosis

The study was conducted to determine the relationship between rotator cuff (RC) tear and the orientation of the glenoid. Ninety‐six shoulders (94 patients) that underwent open RC repair were grouped according to the type of tear. They measured on MRI the acromio–glenoid angle (AG) and the supraspinatus fossa glenoid angle on the anterior–posterior (SGAP) and axial (SGAX) views. RC patients had a smaller AG angle and a larger SGAP angle compared to controls. It was also found a highly significant difference in glenoid version measured by SGAX between anterior cuff tears and posterior cuff tears. Furthermore, an association between RC tear and the orientation of the glenoid relative to the axis of the supraspinatus fossa was identified. Rhinorrhoea, as the name implies is the excess drainage from nose, ranging from clear watery fluid to thick mucus. It can be a part of an acute infection like common cold or a chronic condition like tuberculosis. In severe cases, even a blood tinted mucus may be present. It can happen due to a lot of reasons. The process of allergen sensitization involves the participation of antigen-presenting cells, T lymphocytes, and B lymphocytes and depends on environmental allergen exposure. Sensitization results in the generation of allergen-specific IgE that circulates in the peripheral blood and attaches itself on the surface of all mast cells and basophils including those that home to the nasal mucosa. Subsequent nasal exposure to allergen activates these cells and, through the release of the classic mediators of the allergic reaction, produces acute nasal symptoms. Over a short period of time, these symptoms become indolent, whereas inflammatory and immune cell infiltrate, including eosinophils, basophils, neutrophils, T lymphocytes, and monocytes, characterizes the late stages of the allergic response. In parallel, and probably as a result of the development of mucosal inflammation, the nose becomes primed to allergen and reacts more vigorously to subsequent allergen exposure but also becomes hyperresponsive to irritants and to changes in atmospheric conditions.

A randomized, double-blind, placebo-controlled trial was done to assess the efficacy and safety of mepolizumab versus placebo for severe bilateral nasal polyposis. Patients aged 18 to 70 years with recurrent nasal polyposis requiring surgery were chosen. Patients received 750 mg of intravenous mepolizumab or placebo every 4 weeks for a total of 6 doses in addition to daily topical corticosteroid treatment. A significantly greater proportion of patients in the mepolizumab group compared with the placebo group no longer required surgery at Week 25. There was a significant improvement in nasal polyposis severity VAS score, endoscopic nasal polyp score, all individual VAS symptom scores, and Sino-Nasal Outcome Test patient-reported outcome score in the mepolizumab compared with placebo groups. In patients with recurrent nasal polyposis receiving topical corticosteroids who required surgery, mepolizumab treatment led to a greater reduction in the need for surgery and a greater improvement in symptoms than placebo. It is an eye condition that causes brief attacks of temporary blindness or visual problems like flashing lights in one eye. Diagnosis There are no diagnostic tests to confirm retinal migraine. Diagnosis is accomplished by reviewing the patient’s personal and family medical history, studying their symptoms, and conducting an examination. Retinal migraine is then diagnosed by ruling out other causes for the symptoms.

Scars can result from accidents, diseases, skin conditions such as acne, or surgeries. Some scars can be managed by therapies to an extend but most of them are incurable. Scars come in many shapes and sizes. Cicatrix-when this scar first appears it tends to be pink to reddish in colour and slightly raised. It may also be itchy or painful. As skin continues to heal, the scar will flatten and change colour. In time, many flat scars are nearly the same colour as skin. A flat scar may also be paler or slightly darker than surrounding skin. Hypertrophic scar-This scar rises above the surface of skin. Atrophic scar- below the surface of skin or depressed. Keloid- This scar rises above the surface of the skin. Unlike other raised scars, keloids grow much larger than the wound that caused the scar. Causes
A scar forms when body heals an injury like burns, cuts, etc. Some skin conditions like Epidermolysis bullosa and hidradenitis suppurativa, can also cause scars.
Pathophysiology
Scarring is a natural part of the healing process after an injury. Its appearance and its treatment depend on multiple factors. When the deeper layers of skin are injured, cells make collagen to repair the wound. Because body makes this collagen quickly, it’s thicker and less flexible than the rest of skin. The study was done to evaluate long‐term patient‐reported quality of burn scars. Adults with a burn center admission of ≥1 day between August 2011 and September 2012 were invited to complete a questionnaire on long‐term consequences of burns. The vast majority reported at least minor differences with normal skin (POSAS item score ≥2) on one or more scar characteristics and 78.9% of the patients’ overall opinion was that their scar deviated from normal skin. Patients with severe burns had higher POSAS scores, representing worse scar quality, than patients with mild/intermediate burns, except for color, which was high in both groups. A longer hospital stay predicted reduced scar quality in multivariate analyses. In addition, female gender was also associated with a poorer overall opinion of the scar. In conclusion, this study provides new insights in long‐term scar quality. Scars differed from normal skin in a large part of the burn population more than 5 years after burns, especially in those with severe burns. Recurrent painful round or ovoid mouth ulcers are seen with inflammatory halos. They are more common in females and non-smokers, usually appearing first in childhood and tend to reduce in number and frequency by age. Mouth ulcers, as they are called as in common usage, are largely annoying and recurrent disease, sometimes developing as a hint to the underlying condition. Though not a fatal condition or medical emergency, mouth ulcer needs to be diagnosed and treated well because if not treated they can cause deficiencies and other complications due to lack of food intake. Sometimes, like in chemotherapy, mouth ulcers develop as the side effect of treatment which should be managed differently. SIGNS AND SYMPTOMS Typically seen on the buccal mucosa, the labial mucosa, the floor of the mouth or the tongue. Severe pain or burning sensation develops one day before the actual development of ulcer. Painful lesions with well-defined margins and shallow necrotic center. Yellow-greyish membrane at the base of the lesion and surrounded by raised margins and erythematous haloes. The pain is severe and gets aggravated on eating, swallowing and speaking. The pain usually persists for three to four days. CAUSES Idiopathic Gastrointestinal diseases-Inflammatory bowel disease, coeliac disease Infection- Viral: HSV, HIV, Coxsackie; Fungal: candidiasis; Bacterial: syphilis, Tuberculosis Systemic disease- Reactive Arthritis, SLE, Trauma- dentures Neoplasia- Squamous cell carcinoma Drugs- Chemotherapy:antimalarials Skin disease- Pemphigus, Lichen planus PATHOPHYSIOLOGY The complex interactions of various etiological factors together can trigger ulcer formation. Etiological factors can be classified into predisposing factors and precipitating / triggering factors. The factors like HLA associations, immune dysregulation, nutritional deficiency, personality type A are the predisposing factors. Microtrauma, infections, stress could be the initiating or triggering factor for ulcer formation. Those individuals who are susceptible when exposed to the triggering factors for certain duration tend to develop ulcers. Based on the intensity and duration of the triggering factors, ulcer starts growing till the factors are removed. Pain suffered by the patients is directly proportional to the size of the ulcer and severity of the triggering factors. For example, the serum cortisol level: which is a biomarker of the stress was increased in the subjects with RAS and the increase was directly proportional to the ulcer size. DIAGNOSIS Physical examination – confirmatory in most of the cases A fever also suggests the ulcers may be caused by a herpes simplex infection Blood tests – to check for signs of infection and iron and folate deficiency Skin biopsy – a small tag of tissue from the ulcer is taken and examined to rule out neoplasms. TREATMENTS There are no specific therapies. Patients are advised to avoid oral trauma and acidic foods and drinks which cause pain. Topical or systemic corticosteroids may lessen the duration and severity of the attacks. Chlorhexidine gluconate or tetracycline mouthwash, dapsone, colchicine, thalidomide and azathioprine have all been used with variable effect. PROGNOSIS Most ulcers clear up without treatment and do not leave scars, although they usually return. COMPLICATIONS Untreated, mouth ulcers can occasionally lead to complications including: Bacterial infection; Inflammation of the mouth (cellulitis) and Tooth abscess. In chronic cases, food intake is affected both in quantity & quality which can lead to deficiency diseases. Thirty-five patients (52.2%) developed overt manifestations of BD at an average of 7.7 years after the onset of ROU. The frequency of recurrence was 9.8 times per year in progressive cases. From these results, it appears that highly recurrent ROU is a warning signal for BD. Careful examinations of patients, including their minor symptoms, additional laboratory tests, and regular follow-ups by physicians are required for proper diagnosis. Thirty-three outpatients with recurrent oral ulcers were included in the study.The patients used topical 0.2% HA gel twice daily for 2 weeks. A subjective reduction in the number of ulcers and a decrease in the ulcer healing period was observed in 72.7% of the patients. 75.8% experienced improvement in visual analogue scale for pain. Objective inspection of the ulcers showed a reduction of numbers in 57.6% of the patients, and 78.8% of the ulcers showed a decrease in area. Among the inflammatory signs, swelling and local heat were significantly improved after treatment. The study concluded that topical application of 0.2% HA gel seems to be an effective and safe therapy in patients with recurrent oral ulcers. We aimed to determine the differences in lymphocyte subgroups between DS children and the healthy population and to study the pattern and likelihood for recurrent infections and hospital admission due to infection. Our study was carried out in the Genetic Unit of Mansoura University Children’s Hospital, Egypt. The study enrolled 150 DS (DS group) and 100 controls (CG group). They were assessed for recurrent infections (including tonsillitis, otitis media [OM], pneumonia, upper respiratory tract infections [URTI], sinusitis, and gastroenteritis [GE]) and hospital admission due to infections. All patients were subjected to complete blood count and flow cytometric analysis for expression markers of B lymphocytes (CD19), natural killer (NK) cells (CD56), and T lymphocytes (CD3, CD4 and CD8). We found a statistically significant increase in the frequency of URTIs and sinusitis, OM, pneumonia, and hospital admission in the DS group. As regards the type of recurrent infection in DS, it was highest for URTIs and sinusitis. For age groups below 13 years, a statistically significant decrease in all studied CD markers was found in the DS group, while for the 13-18-year-olds, a statistically significant decrease was found in CD4, CD19, and CD56 in the DS group. Non-significant correlations were found between CD markers and recurrent infection and hospital admission. We concluded that lymphocyte subgroups that carry CD3, CD4, CD8, CD19, and CD56 were decreased in DS. Recurrent infections and hospital admission are still striking feature for DS but are not significantly correlated with lymphocyte subgroups
Immunofluorescence was used to explore the dynamics of anti-VSA IgG responses generated by children to (i) primary malaria episodes and (ii) recurrent P. falciparum infections.
Results Consistent with previous studies on anti-VSA responses, sera from each child taken at the time of recovery from their respective primary infection tended to recognize their own secondary parasites poorly. Additionally, compared to patients with reinfections by parasites of new merozoite surface protein 2 (MSP2) genotypes, baseline sera sampled from patients with persistent infections (recrudescence) tended to have higher recognition of heterologous parasites. This is consistent with the prediction that anti-VSA IgG responses may play a role in promoting chronic asymptomatic infections.
Conclusions This pilot study validates the utility of recurrent natural malaria infections as a functional readout for examining the incremental acquisition of immunity to malaria. Reactive arthritis usually targets knees, ankles and the joints of the feet. Inflammation also can affect eyes, skin and urethra. The infection usually occurs days to weeks before the onset of joint pain. In some cases, a preceding infection may only be identified with laboratory testing. Previously, reactive arthritis was sometimes called Reiter’s syndrome, which was characterized by eye, urethra and joint inflammation. Signs and symptoms
Causes Infection by certain bacteria. Most often, these bacteria are in the genitals (Chlamydia trachomatis) or the bowel (Campylobacter, Salmonella, Shigella and Yersinia). Pathophysiology The bacteria induce (cause) arthritis by distorting body’s defence against infections, as well as the individual’s genetic environment Reactive arthritis is an immune-mediated syndrome triggered by a recent infection. It is hypothesized that T lymphocytes are induced by bacterial fragments such as lipopolysaccharide and nucleic acids when invasive bacteria reach the systemic circulation. These activated cytotoxic-T cells then attack the synovium and other self-antigens through molecular mimicry. This is supported by the evidence of Chlamydia trachomatis and C pneumoniae ribosomal RNA transcripts, enteric bacterial DNA, and bacterial degradation products in the synovial tissue and fluid. It is believed that anti-bacterial cytokine response is also impaired in reactive arthritis, resulting in the decreased elimination of the bacteria. It is, however, unclear why such localization of inflammation occurs. The prevalence of HLA-B27 in reactive arthritis is estimated at 30% to 50% in patients with reactive arthritis, although values range widely. In hospital-based studies with more severely affected patients, frequencies as high as 60% to 80% have been reported. HLA-B27 should not be used as a diagnostic tool for a diagnosis of acute reactive arthritis. The presence of HLA-B27 is believed to potentiate reactive arthritis by presenting bacterial antigens to T cells, altering self-tolerance of the host immune system, increased TNF-alpha production, promoting the invasion of microbes in the gut, and delayed clearance of causative organisms. Initially, the dermal histopathological features of reactive arthritis are similar to psoriasis. Examination of the synovial fluid reveals large macrophages, Reiter cells with phagocytosed neutrophils, lymphocytes, and plasma cells. Extensive pannus formation is very rare. Diagnosis
Diagnosis is largely based on symptoms of the inducing infections and appearance of typical musculoskeletal (joint and muscle) involvement. The objectives of this study were to investigate and validate individual variables and to develop a composite score for disease activity measurement in patients with reactive arthritis (REA). In the first cross‐sectional part, the clinical and laboratory evaluation of 45 patients was used to elaborate the most important individual disease activity measures. In the second prospective part, these variables as well as a composite score for disease activity measurement of REA were prospectively validated in 23 patients at two points in time.The following variables emerged as the most useful for the composite measure: number of swollen and tender joints, patient’s pain and global assessment, and C‐reactive protein. Arteries are blood vessels that carry blood from the heart to different parts of body. Raynaud’s sometimes is called a disease, syndrome, or phenomenon. In Raynaud’s disease, smaller arteries that supply blood to the skin become narrow, limiting blood flow to affected areas (vasospasm). Women are more likely than men to have Raynaud’s disease, also known as Raynaud’s phenomenon or syndrome. It appears to be more common in people who live in colder climates. The disorder is marked by brief episodes of vasospasm, or narrowing of small blood vessels or capillaries. Vasospasm of the arteries reduces blood flow to the fingers and toes. Raynaud’s disease causes some areas of the body, such as the fingers and toes — to feel numb and cold in response to cold temperatures or stress. Signs and symptoms During a Raynaud’s attack, the arteries become very narrow for a brief period. As a result, little or no blood flows to affected body parts. This may cause these areas to:
Numb, prickly feeling or stinging pain upon warming or stress relief. Mostly it takes around 15 minutes or more time to restore the normalcy of blood circulation.
Causes
There are two main types of Raynaud’s—primary and secondary. In primary Raynaud’s (also called Raynaud’s disease), the cause isn’t known. Primary Raynaud’s is more common and tends to be less severe than secondary Raynaud’s. Secondary Raynaud’s is caused by an underlying disease, condition, or other factor such as:
Pathophysiology
Vascular tone is controlled by complex interactions between endothelial cells, the muscular layer, soluble mediators and neuronal stimulation. The imbalance between strong vasoconstricting and weaker vasodilatory events is central to the pathophysiology. Some of the underlying pathological mechanisms may be intrinsic disturbances of endothelial or vascular smooth muscle cells; some may be extrinsic, encompassing neuro-regulation and soluble mediators.

Treatments
Treatments include lifestyle changes like avoiding cold temperatures. Dressing for the cold in layers and wearing gloves or heavy socks will help in mild cases. medicines like calcium channel blockers, alpha blockers, prescription skin creams, and ACE inhibitors, and vasodilators. In rare cases, surgery to block the nerves in the hands or feet that control the arteries and Botox injection are needed.
Prognosis
Primary Raynaud’s (Raynaud’s disease) and secondary Raynaud’s (Raynaud’s phenomenon) have no cure. However, treatments can reduce the number and severity of Raynaud’s attacks.
Complications If secondary Raynaud’s is severe, in rare cases, reduced blood flow to the fingers or toes can cause tissue damage. It can lead to skin sores or gangrene.

As a severe form of periodontal disease (gum disease), it harms the pink tissue holding your teeth in place. Periodontitis can cause teeth to loosen or lead to tooth loss. It is a very common condition but largely preventable. It’s usually the result of poor oral hygiene. Brushing at least twice a day, flossing daily and getting regular dental check-ups can prevent the occurrence and greatly improve chances of successful treatment for periodontitis. Signs & symptoms
Causes
Pathophysiology Mostly it starts with plaque — a sticky film mostly caused by and composed of bacteria. If not removed from time to time, this plaque can develop into more serious diseases, like periodontitis: Plaque is formed when the high amount of starch and sugar interacts with the bacteria in the mouth. It can be removed easily but reforms quickly. Cleaning up is needed from time to time in a proper way. If it stays longer in the mouth, plaque hardens to form a thicker substance called tartar which gets deposited under the gumline. It is filed with bacteria and difficult to remove. This can lead to gingivitis, the mildest form of gum disease. Gingivitis is irritation and inflammation of the part of the gum tissue around the base of the teeth (gingiva). Gingivitis can be reversed with professional treatment and good home oral care. But if not treated, it can cause periodontitis, eventually causing pockets to develop between gums and teeth that fill with plaque, tartar and bacteria. In time, these pockets become deeper, filling with more bacteria. If not treated, these deep infections cause a loss of tissue and bone, and ultimately loss of one or more teeth. Also, ongoing chronic inflammation can put a strain on the immune system. Diagnosis
